Become a Member Today

If you are interested in becoming a member, please click the REGISTER button above and you can easily sign up online.  Please fill out your info, put in your insurance info if you have insurance, create a username/password for your patient portal and sign. 

After you sign up, you will log into your patient portal and go to the "Questionnaires" tab and fill out everything in red. Also please enter you allergies, any specialists you see under "Care Team", as well as your preferred pharmacy.  Don't forget to fill out your medical record release form so we can obtain all your old records so we can smoothly transition you into our practice. 

After you fill out all the forms, an office staff will contact you to go over all the registration details, collect the registration fee, and to schedule your first 60 minute new patient visit with Dr. James.  This visit can be virtual or in person.
